Change to Apprentice Requirements

Amendments to Rule 64B12-16.003, Florida Administrative Code, Apprenticeship Requirements and Training Program, and 64B12-16.004, Florida Administrative Code, Termination of Apprenticeship, became effective February 28, 2019. To view these changes, click here.

How does this change affect you as an apprentice or sponsor?

  • The apprentice shall be responsible for the completion, filing, signature and verification of the Apprenticeship Sponsor Form.
  • If an apprentice terminates apprenticeship with a sponsor or the sponsor is no longer providing training to an apprentice, the apprentice shall submit to the Department within 30 days from the date of the termination a completed Apprenticeship Sponsor Form.
  • Failure to comply with the requirements shall subject the apprentice to discipline or denial of licensure.
